WNY Integrated Care (WYNICC)

WNY Integrated Care is a regional network of community-based organizations and allied healthcare partners working together to improve health outcomes and quality of life through integrated care. As a Community Care Hub, WYNICC centralizes coordination across social care and healthcare systems to address Social Determinants of Health, close gaps in care, and improve follow-through on provider recommendations.

Through this partnership, The Alabaster House is better positioned to collaborate within a broader continuum of care—ensuring that individuals we serve can access coordinated social, behavioral, and healthcare supports.
